Healthcare Transition is the change from pediatric to adult healthcare services. This transition can be very complex for youth with disabilities or special healthcare needs. This transition typically takes place between the ages of 12 and 26 years.
About: Transition Talks Tuesdays is a collaborative effort to provide students, families, educators, and community members important information they need to know before a student turns 18 years old. We will cover a number of major areas that need to be considered. We are grateful to be partnering with the three Autism Society Wisconsin Affiliates (Greater Wisconsin, South Central Wisconsin, Southeastern Wisconsin) and the Children's Resource Center-South.
Who should attend? Family members, self-advocates, educators, social workers, therapists, providers, community members – anyone who wants to understand some of the pieces involved in becoming an adult if you have a disability or special health care need.
