Welcome to the Wisconsin Adult Long Term Care study. We are researchers from the Waisman Center. Thank you for sharing your experiences with us. We value your opinions. Your answers will be used to better understand long term care services in your community.
What is the goal of the study?
We want to learn about quality of life for individuals who receive adult long term care supports in Wisconsin. We will give a survey to gather this information. You can take the survey just one time or you can choose to take it once a year for five years so we can access changes over time.
Who will take the survey?
- People who receive adult long-term care support from:
- Community Integration Program (CIP)
- Community Options Program (COP)
- Family Care
- IRIS
- Partnership
- Family members of adults who receive long-term care supports
What will you do?
You will take an online survey. It will take 20 minutes to complete. You will take the survey one time. You then will have a chance to take the survey again each year for five years if you like. You do not have to take the survey more than once unless you want to do so.
Your name will not be shared. Your responses will not change any long-term care support that you currently receive. We value your opinions. We want to know what is happening in our state. The following survey topics will be addressed:
Survey Topics:
Quality of life
Health
Satisfaction with services (personal care, transportation, etc.)
Daily activities and supports (work, community activities, etc.)
Personal choice and control over services
What are the risks and benefits?
There are no risks to participating in this research beyond those experienced in everyday life. There is a minimal risk for breach of confidentiality if you choose to take the survey for more than one year. There are no direct benefits to this study but we believe that what we learn will help to inform future services and supports.
How Do I Participate?
To take the survey, follow the links below. By taking this survey you indicate that (1) you are 18 years of age or older, (2) you are your own legal guardian or the legal guardian for someone receiving adult long-term care, and (3) you give your consent for your answers to be used for research purposes. You do not have to answer any survey questions you do not want to and you can stop the survey at any time. Choosing not to participate or withdrawing from the study will have no effect on the long term care services you receive.
You will be able to receive $20 for your participation in this survey. At the end of the survey, you will be asked for your personal contact information. You will need to provide an email address or mailing address in order to receive your gift card.

You may ask any questions about the research at any time.
